With reference to the notice of even no. dated 15-02-2017 uploaded on the ESIC websites http://esipgirnr.kar.nic. , www.esic.nic.in and www.esichospitals.gov.in/bengaluru-rajajinagar-home in respect of candidates finding place in select list and short listed for verification for appointment to various Nursing and Para Medical Cadres for Karnataka Region on the basis of performance in written online examination held on 21st and 22nd May, 2016, the Competent Authority has fixed the dates for verification of their respective documents. Call letters for verification of documents to the concerned candidates are being sent to their correspondence address filled in online application. The provisionally selected candidates are advised to visit Recruitment Cell, Administration Branch, ESIC Medical College & PGIMSR and Model Hospital, Rajajinagar, Bengaluru, Karnataka – 560010 as per the schedule given below along with the all required documents

The above candidates are advised to bring all original documents along with two sets of self attested Photostat copies of the following documents in support of their eligibility for the respective post
1. Matriculation Certificate showing the date of birth.
2. Certificates in proof of qualification along with marks sheet.
3. Cast Certificate (Candidates belonging to OBC Category are required to submit latest OBC Certificate in the prescribed proforma of Govt. of India along with form of declaration at the time of verification)
4. Experience Certificate wherever required.
5. Disability Certificate in case of person with disability (PWD)
6. Candidates who are in Govt. Service (including ESIC Employees) are required to produce No Objection Certificate at the time of verification. And seeking age relaxation as Government Employees/ESIC Employees shall have to produce Service Certificate at the time of verification.
7. Two recent passport size photographs.
